Except that the Shining River has turned green, and cattle are dying. Except that the Elf King of Laughing Hollow, a place where no human dares go without fearing for his life, is asking the militia for help. Except that strange substances are oozing from the fissures caused by a recent earthquake. Except that the earthquake also has opened an entrance in the cliffs around the Laughing Hollow that might lead to the fabled dwarf mines of Illefarn. The kuo-toa are a piscine race with a psychic affliction that makes them behave erratically and invent gods when feeling threatened. That same affliction gives them the power to bring those gods to life. Blibdoolpoolp is the most widely worshipped, and thus most powerful, of their gods. Blibdoolpoolp views the kuo-toa as her children despite having been born after them. Like all good mothers, she nurtures and protects. One of her chief concerns is understanding the kuo-toa’s psychic affliction, which causes them to undermine themselves whenever they strive for greatness. She discovers that a corrupted elder brain deep in the Underdark has a connection to the race; though the connection gives them psychic powers, it also turns them into powerless thralls, explaining their self destructive behaviors. Constrained by the same leash on her children, Blibdoolpoolp seeks adventurers to destroy the elder brain. Its death would free the kuo-toa, but potentially at great cost: if the kuo-toa lose their powers, will Blibdoolpoolp die?

A goblin warband has found the secret tunnel which leads from the dwarven keep of Hearth-Home to the nearby hills. The tunnel is intended to be a means of escape if the keep ever falls to attackers, but now it has provided goblins with an easy way into the Underkeep. So far, the goblins have confined their attentions to the various underground rooms that make up the Underkeep, but sooner or later they will find a way to the surface. They pose a serious threat to Hearth-Home - the goblins must be cleared out of the Underkeep so this breach in security can be repaired before the orcs hear of it and overrun the dwarven keep. This adventure is slightly more challenging than Red Hand Trail, and it is recommended that it is played after that quest and before Palace of Dread.
