The adventure begins with the characters discovering the recently murdered Returned, Varyas, and confronting his killers. Varyas's gold mask bears a code on it, one that points toward the sage Khea who lives in the nearby polis. When the characters investigate, they learn from Khea that the writing is undecipherable except for the first line, which refers to the legendary Court of Orestes, a site sacred to the god Phenax. She also notes that a recent earthquake has uncovered a cave along the Khystonos River that could be the secret site's location. If so, then perhaps within lies the key to deciphering the mask, along with its own treasures. With the information provided by Khea, the adventurers set forth into the wilds, where they'll face multiple storied dangers before reaching the Court of Orestes. Once at the lost holy site, the characters discover ancient guardians, as well as an unliving entity with the potential to reveal Phenax's lost secret.

The hobgoblins were not the only forces that remained after Kalarel was slain. Before the Keep’s portal was fully sealed, evil spirits from the Shadowfell escaped into the Nentir Vale. They were drawn to Kalarel’s own dark soul, which remained in the mortal world thanks to a spirit-orb the priest had created to prevent him from moving on after death. The presence of these malignant spirits caught the attention of a frost witch called the Rime Mistress and her followers—unaligned territorial beasts—who rose up to defend their domain. The Rime Mistress knows that the spirits present a greater threat than the hobgoblins do, so she and Prang, the Iron Gauntlet leader, have agreed to an uneasy truce. Sarkt is an illithid sorcerer. It was cast out of its community for its heretical studies (Sarkt was in fact, lucky to escape alive). It didn't take the cunning creature long to pick up a few thralls and find a new place to live, safe from the murderous intentions of its companions. Sarkt dwells in the subterranean ruins of an ancient shrine to Blibdoolpoolp, the crustacean-headed deity of the kuo-toas. This shrine lies on the fringe of a region controlled by kuo-toas, but since Sarkt's arrival, the fishmen avoid the area. A few unfortunate kuo-toas now serve Sarkt as thralls and maintain the illusion that the shrine serves Blibdoolpoolp.
