Long ago, a shapeshifting demon called "Umbraeus, the Defiler" sought dominion over the world. It clawed its way out of the Hells and took control of the region through a mixture of trickery (empowering people it could use) and destruction (raining fire, destroying crops and rivals, etc.). Umbraeus reigned for many years... until it was betrayed by a rival, which allowed a group of adventurers and mages to defeat and trap it in a magical green gemstone. The gem was then buried deep underground, protected by a set of Trials that allowed only those pure of heart and capable of handling its influence to visit it. But a long time has passed. The seals that held Umbraeus at bay have begun to weaken. And the demon has found a way to crack the gemstone. Its malevolent influence has begun to seep out into the world above. The city is plagued by an affliction being called "stone sickness" or "the gorgon’s touch" that disorients people and turns them to stone. Those with, or suspected to have, the affliction are being banished from the city. Some demand a cure, but most are just scared for their loved ones. A ravenfolk woman named Spinel Larkdon, mother to a child with the gorgon’s touch, begs the PCs for assistance. An artifact known as the Shroud of Tiberesh, capable of curing any sickness, is locked away within The Umbers' vault of spoils below the city. Passionate, she is determined to save her son and all those afflicted. Fortunately for the player characters, completing the Umber’s Gauntlet alive means they are not only entitled entrance into the cult, but also a single item from its vault of spoils. The PC's only hope of procuring the Shroud is by traversing this initiation Gauntlet – a series of traps, monsters, and puzzles devoted to the demon-god Nakresh - and claiming the Shroud as their prize.

All the children of the Blight know the nursery rhymes about Bloody Jack Carver, cautionary tales for naughty or overly inquisitive children to mind their manners and obey their parents. However, their parents know the true horror of those times 30 years ago when the lunatic serial killer known as Bloody Jack Carver stalked the fog-shrouded streets of the Blight and abducted children. The killing spree finally ended, but the perpetrator was never caught. When the PCs are deputized to assist in a homicide investigation, they find terrifying clues that point to the three-decade-old Bloody Jack killings and signs to indicate that they were just the beginning. Now the PCs are in a race against time across the breadth of the decrepit and deteriorating city that is the Blight as they attempt to stop a new killing spree before it can start. The PCs’ investigation takes them from the halls of the Capitol and the seedy streets and alleys of Toiltown to the garish carnival piers of Festival and the pollutant-crusted banks of the Great Lyme River. Only they stand between the children of this decayed city and new nursery rhymes being written in their blood. Bloody Jack is a stand-alone adventure set in the Blight for 4–5 5th-level characters.

All Eyes on Chult is a 2–3 session adventure for player characters of 1st level. These characters will rise to 3rd level by the end of the adventure. It takes place in Port Nyanzaru, Chult just after two ships flying black flags enter the port under cover of dark. Though this takes place in locales familiar to players of the Tomb of Annihilation storyline and can be played as a side quest in Port Nyanzaru, it has no relation to the Death Curse nor any other major plot elements of Tomb of Annihilation. Instead, it focuses on two nefarious Faerûnian factions—the Zhentarim and the Xanathar Guild—working together to plunder Chult for their own gain.
