The kuo-toa are a piscine race with a psychic affliction that makes them behave erratically and invent gods when feeling threatened. That same affliction gives them the power to bring those gods to life. Blibdoolpoolp is the most widely worshipped, and thus most powerful, of their gods. Blibdoolpoolp views the kuo-toa as her children despite having been born after them. Like all good mothers, she nurtures and protects. One of her chief concerns is understanding the kuo-toa’s psychic affliction, which causes them to undermine themselves whenever they strive for greatness. She discovers that a corrupted elder brain deep in the Underdark has a connection to the race; though the connection gives them psychic powers, it also turns them into powerless thralls, explaining their self destructive behaviors. Constrained by the same leash on her children, Blibdoolpoolp seeks adventurers to destroy the elder brain. Its death would free the kuo-toa, but potentially at great cost: if the kuo-toa lose their powers, will Blibdoolpoolp die?

A young black dragon and two hatchling dragons live in an excavated sandstone burrow in the swamp west of Knacker Knob. Connected to the dragon lair is a complex of sandstone tunnels and chambers inhabited by lizard men. Some sandstone tunnels and chambers are partially flooded, and others are filled with nasty things such as slimes, oozes and gelatinous cubes. The lizard men revere the dragons as magical spirit guardians. In small groups the lizard men are easily defeated, but if allowed to prepare a common defense, the lizard men tribe with its spellcasters may present serious threat to the player characters (PCs). "The Hive" is a spawn of Arbeyach lair, also featuring giant termites and termite swarms, for five 6th-level characters. This adventure can be finished in one session. Several mounds in the Forsaken Hills are the dens of voracious termites that have added flesh to their diet to supplement dwindling wood supplies. At night, swarms of termites hunt and devour prey. They swarm over victims in wooden carts and wagons, and bring the dismantled wood back to their mounds. Three spawn of Arbeyach, searching for a location to summon Ia'Affrat, Arbeyach's herald, discovered a mound filled with particularity nasty termites and used their vermin empathy to gain acceptance. They dug out a cavern beneath the mound, and now use the termites to protect themselves and gather food while they complete a difficult ritual involving dozens of sacrifices. The final sacrifice is prepared and the spawn are finishing their ritual when the party arrives.
