The Bleeding Hollow was written as a tribute to the golden era of adventures. Danger lurks around every turn, and a great over-arching storyline ties everything together. There is much to discover and learn, and solving the woes of the adventure is entirely up to the players. They will choose how to deal with the myriad challenges put forth, and will probably run down a red herring or two. They might choose a very dangerous path unknowingly and pay the consequences fortheir actions. That is intended. Let the story lead your party, and your players lead the game. You won’t regret it. A Pathfinder Roleplaying Game adventure for 15th-level characters, this volume of Pathfinder Adventure Path is part 6 of 6 of the Kingmaker Adventure Path, in which the heroes win and defend a small kingdom from threats foreign and domestic. PCs should advance to 17th level by the end of this adventure. This adventure begins after the War of the River Kings has ended—the PCs, be they the war’s victors or losers, are faced with the task of rebuilding their kingdom or perhaps helping to rebuild Pitax. Yet soon after the war is over, a new peril strikes the region as strange monsters and violent bursts of rapid vegetation growth and bizarre weather plagues the Stolen Lands. At the same time, the PCs learn that one of the treasures discovered in Pitax’s House is a nascent vorpal sword, a weapon of immense power, and as the strange weather and blooms of life and monstrous incursions increase, so does this sword’s intelligence and power. The PCs soon learn that their kingdom faces an invasion, but this time not from the physical world. Some fell force from the legendary First World is attempting to expand into this world, and when the PCs begin to fight back against these verdant blooms, they discover that it’s possible to step from this world into the First World realm of Thousandbreaths. There, the PCs face powerful new threats unlike anything they’ve seen before, and learn that the dangers facing their kingdom are even greater than they feared, for the fey ruler of Thousandbreaths is about to absorb the Stolen Lands into a bauble for her own purposes, an act that would scour clean the region and leave behind nothing but a wasteland. If the PCs hope to save their new kingdom from this threat, they must combat the nymph queen Nyrissa both by stopping the various blooms in their kingdom and by traveling into Thousandbreaths itself to confront the dangerously insane nymph.

This quest is for a party looking to resurrect a character whose corpse is missing body parts necessary for survival. Lower-level resurrection spells don’t work on a creature that is missing a head, heart, or other vital organ. To resurrect the body, the characters turn to Vaprak, patron deity of trolls, and attempt to earn the god’s assistance. However, a band of frost giant hunters intend to destroy Vaprak's temple along with everyone inside it. In the search for allies in the Underdark, you are called upon to travel through the recently opened Waydown sinkhole to find the lost myconid colony of Sporedome. It is said that they once cultivated a strain of mushrooms that could protect one from falling into madness. Can this be the cure for the rapidly spreading onslaught of insanity?
This adventure is written with the intent that it takes place in the subterranean world called the Underdark in the world of Forgotten Realms, although it may be placed in other settings. Within the Underdark, Cyrog's lair can be placed anywhere though it is stated to be thousands of miles east of Gravenhollow. If you are running Out of the Abyss, you may want to have your players see the vision Cyrog Lives! Hail Orcus! (OOTA, 158). Additionally, it is recommended that this adventure be used for a party of 4 to 6 players, level 9 to 12. While merely hinted at through visions in Out of the Abyss, Orcus is roaming the Underdark unchallenged with an army of mind flayers at his disposal. Cyrog, the elder brain of a mind flayer colony in the Underdark has died. The demon lord Orcus has reanimated and taken control of it. The hive of mind flayers is now bent to Orcus's will through Cyrog as he strives to make all life in the multiverse into a vast undead horde under his control. The adventurers are contacted by a patron who gives them a special item (such as brain worms) to suppress their brain waves and make it possible to infiltrate the Cyrog's Lair without immediate detection. The adventurers must defeat or otherwise subdue mind flayers and a few guards, override the elder brain's mechanical defense system, and slay Cyrog.
