Beyond the mortal world, somewhere outside the bounds of the Astral Sea, the Far Realm yawns immeasurable. Beyond the reality of gods and mortals, this plane is a realm of contradictions, a mad place of fevered creation checked only by pointless extinction. The Outside is a sea of diseased possibilities, where the stuff of madness seethes in a soup of the incomprehensible and the unnamable—shuddering, squirming, and sentient. PCs can come to the town’s defense and repel the horde. A priestess of Ioun then turns to the heroes to find the item the monsters were seeking. From the recovering town, the heroes cross the wilderness to the Kaorti Temple. The trail ends at a ruined tower overlooking a deep ravine. Inside the tower, the heroes must overcome the terrible creatures in their search. In the end, they find the relic has been stolen.

The PCs fight their way into deep ruins, defeat the demon lord’s toadies, and retrieve the Sun’s Sliver. Once the PCs have recovered the Sun's Sliver, they must confront the Winter Witch in Winter's Heart, the witch's icy sanctum in the Feywild. Only there can they confront the cold-hearted archfey and save a section of the world from a wintery doom.

In what became known as the Age of Madness, an astral lich calling himself the Crimson Hand descended from the sea of stars, setting his fell gaze upon Gaea. Her red moon he made both his temple and his phylactery, and from there he sought to enslave those who would be ruled and devour those who would not. The Ancients of the four corners of Gaea united their knowledge against this threat, constructing a colossal golem, and inscribing the four POWER WORDS upon four scrolls. These scrolls they fed to the colossus, who ascended to the crimson moon in an instant. The sages of the realm watched as the flashes of battle signaled in the night sky, ending as the crimson moon itself split in half. The lich and the colossus were destroyed, and the Age of Madness was ended. Millennia have passed, and now a new threat rises from the netherworld. The four POWER WORDS have been forgotten, lost among the shattered remnants of the moon. The sages have once again united what remains of their power, this time to teleport a paltry handful of crusaders to retrieve the lost scrolls before the world is doomed.

Darkness looms near Winterhaven! When the party investigates, they will discover that the kobold's leader, Irontooth, bears a tattoo of a ram-headed demon. This is a troubling portent, for the townsfolk will report that this can only mean the dread god Orcus, master of death, has an active cult in that ruined keep! It is up to the party to enter the Keep on the Shadowfell and put a stop to the cult's fell machinations before it threatens all the realm. There they will fight the evil priest Kalarel, Scion of Orcus and seal off the vile portal to the dark realms he is master of. Should the heroes seize victory, glory and treasure are sure to be theirs. But first they must endure the challenges of that dreaded KEEP ON THE SHADOWFELL!
