This adventure is written with the intent that it takes place in the subterranean world called the Underdark in the world of Forgotten Realms, although it may be placed in other settings. Within the Underdark, Cyrog's lair can be placed anywhere though it is stated to be thousands of miles east of Gravenhollow. If you are running Out of the Abyss, you may want to have your players see the vision Cyrog Lives! Hail Orcus! (OOTA, 158). Additionally, it is recommended that this adventure be used for a party of 4 to 6 players, level 9 to 12. While merely hinted at through visions in Out of the Abyss, Orcus is roaming the Underdark unchallenged with an army of mind flayers at his disposal. Cyrog, the elder brain of a mind flayer colony in the Underdark has died. The demon lord Orcus has reanimated and taken control of it. The hive of mind flayers is now bent to Orcus's will through Cyrog as he strives to make all life in the multiverse into a vast undead horde under his control. The adventurers are contacted by a patron who gives them a special item (such as brain worms) to suppress their brain waves and make it possible to infiltrate the Cyrog's Lair without immediate detection. The adventurers must defeat or otherwise subdue mind flayers and a few guards, override the elder brain's mechanical defense system, and slay Cyrog.

The city is plagued by an affliction being called "stone sickness" or "the gorgon’s touch" that disorients people and turns them to stone. Several days ago, a colony of myconids were born from the gloom and rot of a swamp near a small fishing town. They found a place to form their new colony, and settled on a crumbling mausoleum in a long-forgotten graveyard. Their rapport spores have been spreading unchecked, and have begun to infect the dead buried underneath the marsh. These dead have risen as spore servants, which the myconids are using as watchdogs to protect their new home from unwelcome intruders—like the adventurers. In this encounter, the characters stumble upon an old country graveyard in the middle of a swamp. When they investigate, they fall under attack by the living dead! Once they fend off their undead assailants, an investigation of the mausoleum reveals the true villain behind the attack: a group of mycanoids that infested the corpses with their spores, turning them into telepathically controlled puppets. From here, the characters can either negotiate with the mycanoids or kill them. This encounter’s three acts are: The Discovery: the characters find an abandoned graveyard. The Trial: the characters are attacked by undead. The Revelation: the characters learn that the zombies are really mycanoid spore servants, and can try to reason with the mushroom-people or destroy them. Either way, there’s treasure!

The Children of the Harvest is a stand-alone adventure set in The Blight for 4—6 7th- to 8th level characters. The Blight is a dark place. Children disappear all the time, especially those of poor. The Thorn Hag, an ancient evil thought vanquished by the Elf Queen and her heroic companions many years ago, has arisen from the dead. A fey harp, created from the heart of the treant that perished in the battle, has gone missing from Satyrs' Glen and the Thorn Hag seems to be behind it.
