A young black dragon and two hatchling dragons live in an excavated sandstone burrow in the swamp west of Knacker Knob. Connected to the dragon lair is a complex of sandstone tunnels and chambers inhabited by lizard men. Some sandstone tunnels and chambers are partially flooded, and others are filled with nasty things such as slimes, oozes and gelatinous cubes. The lizard men revere the dragons as magical spirit guardians. In small groups the lizard men are easily defeated, but if allowed to prepare a common defense, the lizard men tribe with its spellcasters may present serious threat to the player characters (PCs). Mystery of the Nightmare Venom is the second adventure in the Nightmare Venom campaign for the prehistoric D&D Planegea campaign setting. It is designed for four to five 2nd-level characters using the D&D 2014 ruleset. In the first campaign adventure, Venom Attack!, the PCs encountered an envenomed spider-raptor sent by the Spiderfolk Queen to attack Edgegather, while a minion of a green hag sought to collect a sample of the venom. In Mystery of the Nightmare Venom, the PCs begin to investigate this strange magical poison at the behest of the Venom Guard: A vengeful green hag seeks to corrupt the inhabitants of Edgegather. Can the PCs stop her before her poison brings the settlement to its knees? The goblins of the Unfalls may have useful information but the PCs must first retrieve a lost relic from the spiderfolk of the Venom Abyss before they can ask their questions. The PCs investigate a vast and ancient temple beneath Edgegather where the mysterious Veiled Goddess can point them towards the Oracle of Seerfall. Mystery of the Nightmare Venom contains five new monsters (the envenomed chattercrest, the envenomed Venom Guard, the cauldronborn, the Glassback spiderfolk, and the faithful remnant) and two new magic items, the bloodtallow candle and the obsidian centipede.

The people of Feycircle believed the fairy ring for which their town was named protected them from the encroaching sands of the Western Wastes. When the ring withered, they learned they were right. Feycircle’s dewflecked pastures and verdant forests succumbed to the Wastes in an instant. Feycircle sank deep into the sands and was soon occupied by a tribe of fanatical dust goblins, a herd of dogmoles, and the giant worms that caused the blight. The PCs may explore the keep—the only structure remaining in the dusty sinkhole now called the "Pit of the Dust Goblins." Inside the keep is an entrenched gatekeeper who can divulge that two children remain in town, imprisoned by the goblins. The dust goblins have summoned and imprisoned a selang—a shadow fey—inside the blighted fairy circle. The goblins fear open combat and have laced the sunken keep with traps, and would sooner parley than fight.

In times ancient, an infamous paladin-artificer named Khaldun unearthed a cursed tome bound in the flesh of the first medusa. Driven to madness by its secrets, he used the necromantic magic contained within to call forth an army of dread warriors from the Netherworld. Though his assault was devastating, he was ultimately unsuccessful, and the mightiest crusaders of the realm banished Khaldun to the Netherworld, where he would rot for eternity at the bottom of the River Dis. Unfortunately, rather than perishing in that bleak plane, Khaldun escaped his silt grave and thrived, amassing a legion of undead followers and erecting a Tomb in which he could perfect his mechanical craft without interruption.
