Modrons, Mephits & Mayhem is a Dungeons & Dragons 5E tier 2 adventure for three to five characters of levels 5-8. It is primarily set in a modron-designed research facility that has been abandoned by its creators but retains guardians that are still active. This set included a 48-page rulebook covering the first three levels of play, and was skillfully edited by 𝐃𝐫. 𝐉. 𝐄𝐫𝐢𝐜 𝐇𝐨𝐥𝐦𝐞𝐬 from the original 1974 D&D rules written by 𝐆𝐚𝐫𝐲 𝐆𝐲𝐠𝐚𝐱 and 𝐃𝐚𝐯𝐞 𝐀𝐫𝐧𝐞𝐬𝐨𝐧. Holmes advanced the "Sample Dungeon" concept by writing a new thematic dungeon with a strong backstory, creating an adventure that has remained a fan favorite over the decades. Officially, its only title is "Sample Dungeon" but colloquially it goes by various names based on Zenopus, the doomed wizard who built the dungeon under his tower. The Therayle farm, which has been in the family for generations, was the best producing in the region. It now faces ruin. Hugh Therayle, the current owner, is at wit's end trying to reestablish even a modicum of control over the disrupted land. But, when Hugh saw the ancient doorway jutting up from the middle of the field where his corn used to be, he knew that this wasn't the type of problem a plow could solve. More than two centuries ago, a red wizard by the name of Houn made a discovery that shook the foundations of magic on Toril. Yet, due to a considerable amount of effort on his part, it never saw the light of day. The exact nature of this discovery is unknown but it involves the location of the skeletal fragments of an ancient being. The weave, which governs all magic on Toril, reacts strangely to the fragments. More information existed before Houn destroyed his research and fled Thay. His current whereabouts are unknown but many of his peers have attempted to track him down and steal his research for themselves.

Somewhere under a lost and lonely hill of grim and foreboding aspect lies a labyrinthine crypt. It is filled with terrible traps and not a few strange and ferocious monsters to slay the unwary. Accounts relate that it is quite unlikely that any adventurers will ever find the chamber where the demilich Acererak lingers, for the passages and rooms of the Tomb are fraught with terrible traps, poison gases, and magical protections. Furthermore, the demi-lich has so well hidden his lair, that even those who avoid the pitfalls will not be likely to locale their true goal. In the far reaches of the world, under a lost and lonely hill, lies the sinister TOMB OF HORRORS. This is an adventure for a party of level 4-6 designed to showcase a number of newly created Oozes in the Beyond the Black Pudding compendium. The party is brought in to help figure out what to do about a giant ooze that is threatening to engulf a small town. They discover that it is being called by a magical beacon somewhere nearby, and by following a small ooze they are able to find where the signal is coming from. It turns out to have been set off by The Visionary, an intelligent Vitreous Humor (a kind of ooze beholder) with dreams of power who discovered it in an ancient temple to Juiblex. Unfortunately, having turned it on, The Visionary had no idea how to turn it off again and is now barricaded in the depths of the temple, assaulted by oozes on all sides.

With your first set of adventures firmly under your belt you are relaxing and basking in the glory of a job well done. While at the local tavern imbibing in some well-earned spirits, a commotion can be heard outside. As you step outside the villagers are pampering two children who apparently just rode into town on a very sweaty and tired pony. An arrow is sticking out of the mount’s flank. As the animal is removed the townspeople pepper the children with questions as both are obviously afraid. It would appear the drinks will have to wait.
